There are always a few contraindications to the treatments offered by Five Acupuncture, and they don’t always mean that a treatment cannot take place. Your initial consultation is incredibly thorough and it is very important that you inform your practitioner of any medical conditions you have and those you have suffered with in the past, and provide updates at subsequent sessions if any of the discussed aspects change, this is especially the case with any changes to medication. It is your responsibility to check with your medical practitioner in case you suffer from a condition and/or are in doubt whether to have treatment.

PROTECTION OF YOUR PERSONAL DATA

During your initial consultation and treatments, you will provide personal information relevant to maintaining a patient-practitioner relationship and used for tailoring your treatment. This information will be kept strictly private & confidential, stored securely and maintained accurate by updating it as advised by you.

You may request that we issue a copy of the personal information held about you. Provision of such information will be subject to the payment of a processing fee (currently £15.00) and two forms of identification, within 10 days from receipt of suitable forms of identification. Five Acupuncture may withhold such personal information to the extent permitted by law.

APPLICABLE LAW

These terms and conditions are governed by and will be interpreted in accordance with the laws of England and Wales to the exclusive jurisdiction of the Courts of which countries the parties agree to submit.

Five Acupuncture reserves the right to change these terms and conditions at any time by posting new terms and conditions on this site.
